Round Ligament Pain During Pregnancy

That sudden, sharp grab in your lower belly can be alarming the first time it happens. It's called round ligament pain — and while it's normal, you don't have to simply endure it.

What Is Round Ligament Pain?

The round ligaments are two thick bands of tissue that run from each side of the uterus, down and forward through the inguinal canal (the passage in the lower abdominal wall), to the labia. Their job is to help support the uterus as it grows.

During pregnancy — especially the second trimester when growth really accelerates — these ligaments stretch and thin as the uterus expands. That stretching can produce sudden, sharp pain in the lower abdomen, groin, or hip area. It often feels like a pulled muscle or a quick stabbing sensation.

The pain is usually brief but can be quite intense and alarming if you don't know what's causing it. While round ligament pain itself is a normal part of pregnancy, a poorly aligned pelvis makes it significantly worse — and that's where chiropractic care can help.

Why It Happens — and Why It Can Get Worse

Rapid Uterine Growth

The uterus grows from roughly the size of a pear to the size of a watermelon over nine months. The round ligaments have to stretch dramatically to accommodate this growth, and they can become taut and tender as a result — especially during growth spurts in the second trimester.

Sudden Movements

Round ligament pain is classically triggered by quick movements — rolling over in bed, standing up too fast, sneezing, coughing, or even laughing. The sudden motion causes the already-stretched ligament to contract sharply, producing that characteristic stabbing sensation.

Pelvic Misalignment

When the pelvis is out of alignment — which is extremely common during pregnancy — the round ligaments can be pulled unevenly. One side may be under far more tension than the other, making that side far more prone to pain. Restoring pelvic balance reduces this asymmetric strain.

Relaxin-Driven Instability

The relaxin hormone loosens ligaments throughout the body, including those that help stabilize the pelvis. When the pelvis isn't well-supported, the round ligaments pick up additional load — putting them under even more chronic tension between episodes of acute pain.

What Round Ligament Pain Feels Like

If your pain is severe, persistent, accompanied by fever, bleeding, or doesn't resolve within a few minutes, contact your OB or midwife right away. These could indicate a different condition that needs evaluation. Round ligament pain is brief — it's not a prolonged cramping sensation.
